White House Clinics now offers an online request tool below that verifies your identity by asking for a photo of your driver’s license, which can be taken via webcam or smartphone. There is no additional charge to use this service.

Use Kentucky Immunization Registry’s (KYIR) secure Public Portal to access and obtain a copy of you or your child’s immunization record.
